Home Health Scribe- RN Preferred

Job Summary:

The Home Health Scribe is responsible for remote documentation of comprehensive assessments performed by Registered Nurses in the home setting, completed in real-time, and maintaining both audio and visual connectivity throughout the assessment. Comprehensive assessments include Starts of Care, Resumptions of Care, Recertifications and Significant Changes in Condition.

The Home Health Scribe is responsible for accurately capturing the assessment findings of the Registered Nurse, accurately scoring the OASIS and developing the comprehensive Plan of Care.

The Home Health Scribe is responsible for transcribing real-time assessment findings and medical information from patient encounters into the electronic medical record, collaborating with the assessing clinician to ensure accurate transcription and completion of all documentation same-day.

The Home Health Scribe reports to the Director of Quality Assurance, with close partnership with URS and agency leadership, and requires independent decision-making, critical thinking and problem solving skills based on a sound knowledge of best practices, conventions and home health regulations. This position is a clinical liaison for all home health branches and partners with local branch leadership to be a cohesive member of the branch clinical team, creating successful relationships with the staff of each branch.


  • Registered Nurse
  • Must have a minimum of 3 years home health care experience
  • Working knowledge of Home Health Regulations, State Regulations, CMS Regulations, Patient Driven Groupings Model (PDGM), Review Choice Demonstration (RCD)/Pre Claim Review (PCR)
  • Proficient in OASIS D1
  • Basic understanding of Coding guidelines
  • Broad-based understanding of standards of practice for clinical documentation
  • Proficient in Point Care and back-office Home Care Home Base required
  • Detail oriented
  • Strong written, oral and interpersonal communication skills

Essential Functions:

  • Works closely with branch leadership and Registered Nurses to coordinate Scribe assessments
  • Communicates effectively with assessing RNs to ensure accuracy of transcribed content and OASIS
  • Completes all documentation in real-time during the assessment visits while on audio and visual Zoom (or equivalent) with the assessing clinician
  • Submits all required documentation the day of the assessment
  • Resolves any URS queries immediately to facilitate timely processing/locking of Plans of Care and OASIS
  • Meets productivity expectations to maximize field RN availability to complete maximum number of comprehensive assessments
  • Ensures all CMS process measures are completed at the time of comprehensive assessment and that appropriate interventions are implemented to address appropriate process measures
  • Remains updated on home care criteria and regulatory guidelines, trends in the home care industry, regulatory and compliance standards
  • Implements approved recommendations to ensure success in correcting clinical record deficiencies
  • Participates in Case Conference, staff meetings and other clinical meetings as required and acts as a resource and educator to clinicians to improve documentation
  • Participates in conference calls as requested, to share best practices and expertise with the team
  • Possible intermittent travel in order to meet objectives
  • Other duties as assigned to meet team/department objectives
