Patient Care Coordinator/Office Manager (LVN)

We are looking for a great Patient Care Coordinator to join our team in Plano, Texas.

General Purpose: Schedule patient services and coordinate with staff. Assume the responsibility of coordination of office functions in accordance with state, federal, and local regulations.

Essential Functions


  • Schedules and documents patient schedules in a timely manner.
  • Participates in coordinating care with management, patient, and interdisciplinary group.
  • Provides effective communication to patients, staff members, other health care professionals, and referral sources.
  • Respond to Agency and patient needs in a professional and creative manner.
  • Demonstrates commitment, professional growth, and competency.
  • Promotes the Agency’s philosophy and administrative policies to ensure the quality of care and a positive image to customers.
  • Responsible to manage all office functions and processes including clerical, personnel, medical records, office machines, and payroll.
  • Assists in the billing process and financial functions as needed.
  • Oversees Agency communications including pages, telephones, mail, and tracking of physician orders.
  • Promotes compliance with all state & federal regulations and stays current with hospice regulations.

Education Requirements:

  • Graduate of an accredited school of vocation/practical nursing preferred. High School graduate.

Preferred Experience Requirements:

  • One year of experience as an LVN/LPN in hospice preferred.
  • Two years of general office management and human resource experience preferred.
